The National Social Care Conference 2025 / NSCC25 in Llandudno was a celebration of the people who create and contribute to the rich tapestry of social care in Wales. As we launched our 25th-anniversary year, we caught up with colleagues, clients and friends, old and new, who have shared in our journey.

From our early days, founded in 1999 by former social worker Gabe Conlon in his humble home office, to becoming a trusted consultancy supporting health, care and public-sector organisations, our story has always been about people. NSCC25 was the perfect place to begin our celebrations.

“At Practice Solutions, our mission is to help people live more meaningful lives by connecting people, fostering innovation and unlocking potential,” explains our CEO, Sarah Day.

The National Social Care Conference gave us the space to share our humanity, rather than simply showing products. Our stand was built on invitation: come and engage, share your story, and join our journey.

Over the conference week, we launched our Library of Inspiration at our exhibition stand. Delegates were invited to pick up a commemorative bookmark, sign up to our mailing list, and enter a competition to win one of 25 inspirational books chosen by our team and associates. The idea behind the library was to create a shared space for inspiration and exchange. It embodied our vision and values: joining the dots between people and organisations, emphasising human value, and amplifying citizen voices.

We wanted to do something different this year. It wasn’t about selling a product , but rather to invite conversation, to reflect on our 25 years and to look ahead. It was about creating moments of shared purpose and connection.
Lewis Gwyther, Head of Partnerships
Our week began with the NSCC Network Members’ Event on Tuesday 21 October, where we as one of the founding partners connected with fellow member-leaders ahead of the main conference. During the two days at NSCC25, we focused on showcasing some of our most innovative work, such as our collaborations on Use of Alcohol in Care Homes and AI and Digital Transformation, and our communications campaign developed with Age Cymru, Open Doors, Connected Lives, which highlights positive experiences of living in care homes.

Behind every inspiring moment at the conference is the exceptional team who made it happen. The conference is a particularly important and special event for us, as not only do we participate as exhibitors, we also help to deliver the conference in our role as Business Delivery Unit for ADSS Cymru.
These two photos [below] mean a lot to me. They capture almost everyone who, for the last nine months, has worked behind the scenes to deliver this year’s National Social Care Conference. Together, we bring a powerful mix of skills – from curating content and managing logistics to using creative flair to engage our audience. The point is: we are greater than the sum of our parts. It’s this blend of expertise, creativity, and dedication, that makes the conference what it is.

For Sarah, a special personal highlight came when her daughter, Jess, joined the team for the week. “For a family-run organisation like Practice Solutions, moments like that matter,” she said. “You never know – maybe in a few years’ time, another generation will be helping to carry the torch.”
To everyone who visited our stand, joined the conversation, or took home a bookmark and a book recommendation: thank you. Here’s to the next chapter of Practice Solutions: celebrating 25 years of connection, creativity and collaboration in Wales.
